Use these forms when applying to SJES.  If you have any questions about the forms or process, please contact our Admissions team at 540-347-3855.

arrowApplication for Admission (online form available February 1)
This is our online application system. A $125 application fee is due at time of submission.

arrowEarly Application Request Form (online form)
Request to apply before admission is open to the public with Early Application. A $150 non-refundable registration fee is due at time of request. If you have a student currently at SJES and want to make an application for a sibling, you are automatically included in Early Application and do not need this form and do not pay a fee. If you are a Saint James' parishioner, please use this form but do not pay the fee.

arrowRecords Release Form (pdf)
Elementary applicants must send this form to their current school to have transcripts sent to SJES.  Records must be received by SJES prior to your student visit and parent interview.

Use these forms if you have been accepted to SJES. Please return all completed forms to the School Office:

arrowSchool Entrance Health Form
Upon acceptance, new elementary students must have their doctor complete this form. This form must be on file at the start of school.  For preschool students, we must have the most current immunization records on file.

arrowExtended Day Application
If you need a longer day at SJES throughout the school year, you may register for our Extended Day programs. Please call the Business Office at 540-347-3855 for rates and enrollment, If you need to drop in for any Extended Day program during the school year, please contact our Extended Day Coordinator via phone or text at 540-860-0681.

arrowEmergency Contact Form
This form must be completed for each student before the first day of school. Parents/guardians must update it as needed.

arrowPhoto Release / Contact Authorization Form
This form is due the first week of school and must be updated by the parent/guardian as needed.

arrowMedication Consent Form
Use this form if your child needs (or may need in an emergency) medication administered during the school day. This form must accompany unopened medications with the Rx label. Please contact the Health & Safety Coordinator at 540-347-3855 if you have questions.

The Parent-Student handbook contains important information and policies pertinent to SJES and is updated every school year, and throughout the year as needed. Parents are required to sign the Acknowledgement Form at the back of the handbook.

Inclement Weather Policy
SJES follows Fauquier County Public Schools on the first day of a weather event. From the second day on, the School will make its own call. 

Elementary Uniform Policy
Kindergarten through Grade 5 wear uniforms daily.
